An honest clinical guide to a transformative class of drug.

Semaglutide and tirzepatide are genuinely transformative when used well. They reduce visceral fat, improve insulin sensitivity, and lower cardiovascular events in patients who needed help that lifestyle alone could not provide. The clinical evidence is real, and the cardiologist who tells you otherwise is not paying attention. The goal of this book, however, is metabolic health, not slimness; some patients will gain muscle and lose visceral fat without ever becoming thin, and the framework counts that as success.

 

Used carelessly, the same drugs strip muscle alongside fat and leave patients weaker than they started. They cause nausea, gastric dysfunction, and, in a small minority, more serious adverse effects. Stop them suddenly and the weight comes back — sometimes with interest. The popular press treats them as a miracle; the prescribing reality is more nuanced than that.

THE PRINCIPLE

This question matters more in this book than in any of the others. A GLP-1 prescription is either a runway or a life sentence — and which one it becomes depends on what you do with it.

The honest answer is that some patients will take a microdose maintenance forever, and that is fine. Others will use the drug as a six-to-eighteen-month runway and taper off cleanly. Both are defensible. What is not defensible is starting the drug with no plan for either route, no muscle-preservation strategy, and no lifestyle changes underneath. That is when the prescription becomes the only thing holding the result together — and the day it stops, so does the result.

When the door is worth opening

The legitimate clinical indications, separated honestly from the marketing. The patient profiles where GLP-1 is genuinely helpful, the profiles where lifestyle changes alone are likely to work, and the smaller group where the drug is contraindicated or unwise. This is the chapter that decides whether you should be reading the rest of the book at all.

Using the drug well, not aggressively

The micro-dosing and titration approach that minimises adverse effects and muscle loss. How to manage nausea and gastric tolerance. What to do if a week's dose doesn't go in. Why the lowest effective dose, held for longer, beats the textbook escalation for most patients.

The lifestyle underneath

The drug suppresses appetite. The suppressed appetite gives you a runway. The runway is for installing the resistance training, the protein intake, the glucotype-aware eating, and the sleep architecture that — if absent — turn the prescription into the only thing keeping the result. The lifestyle is what holds when the drug ends. The lifestyle is also what makes a maintenance microdose genuinely small.

The protein floor, ten minutes of daily resistance training, and the strength tests that tell you the truth that the bioimpedance scale cannot. Much of what DEXA records as "muscle loss" on GLP-1 is glycogen and intramuscular fat; the worrying numbers in the literature need this caveat. What matters is what you can lift and how steadily you can stand — and these can be rising even while the scale says you're losing mass.

The starting dose, the titration interval, and the lowest-effective-dose endpoint. Why slower titration produces better long-term outcomes than the textbook schedule for most patients.

Behavioural integration

Using appetite suppression as the window to install the lifestyle changes from Book 3 — when food is genuinely uninteresting, the wrong food is easier to give up. The runway is short; spend it well.

The planned taper

What coming off looks like. The measurement checkpoints that confirm the lifestyle has held. The signs that suggest a maintenance microdose is the better long-term answer instead.

The book is honest about the fact that some patients should stay on a maintenance dose for life, others should taper off cleanly, and the right answer for any individual is decided by the data — not by a default position taken by either the prescriber or the patient before the data exists.
